Yoga helps with prevention and getting over it. It can be a hard coniitdon to cure.  The podiatrist will probably tell him to take NSAIDs (like Aleve) and an anti-inflammatory (Prednisone) if possible.  In the meantime, buying foot inserts that will help support the arch really helps, too. Or you can get a two-liter soda bottle, fill it up with warm water and cover the cap, and roll under the foot to help support his arches so they don't fall. I think for me, the main issue was a lack of mvemeont/flexibility in my feet. (A stiff big toe is called hallux limitus. A REALLY stiff one is called hallux rigidus!) One stretch I got given was to raise my big toe in a golf ball, with the other toes flat on the floor. And in yoga, we did an exercise in which we manually held down the other toes while lifting the big toe, and then vice versa. (It's amazing how much  willing' your toe to move, thereby improving neuromuscular coordination, helps ) I've also done manual rotations and abductions (pulling away) of my big toe to help improve it's range of mvemeont. Finally, I've been wearing five-toed Toetoe socks and Vibram Fivefingers as much as possible (snow permitting!) to remind my feet they are devised of separate bits and are not just single slabs of flesh and bone! Hope this helps. I will post some of the other strength exercises for other body areas in a future blog.                ]]></description>

For example, I have at least 3 of my patients running the US Women's Olympic marathon trials next weekend that are in graphite or 1/8  poylethylene orthotic decievs.  The shell of the device ends behind the metatarsal heads and the extension to the toes will include posting etc if needed.  I completely agree that most runner's need forefoot posting.As far as the foot becoming weaker with full time orthotic use, it is common sense, not an old wives tale. If you put a cast on someone their muscles atrophy.  There are no scientific studies to prove that orthotics don't weaken the foot either.  I recommend that my patients only wear their orthotic decievs when they run and not during the day. I also encourage the use of the Nike Free, or barefoot running on grass in the form of strides etc in all my non injured running patients.As a matter fact, there are also no scientific studies that prove that custom orthotic decievs alter the biomechanics of the foot at all in terms of alingnment. If you review the medical literature there are very few well designed studies that prove anything scientific about the use of foot orthotic decievs ability to alter the function of the foot at all.Now you and I both know they work, but evidence of the decievs working is far from scientific.Dr Benno Nigg has done the majority of the best research in this area.  One quote from him concerning this research is: The work on foot support related topics is studied for the relationship between comfort and functional aspects of shoes and orthotics/inserts. It seems that the effects of shoe/insert interventions produces only small effects in the alignment of the skeleton but large effect in the reaction of the muscles. This suggests a new concept of a preferred skeletal movement path for a given movement task. Please do not take my posting the wrong way.  I have great respect for your work and reputation and I have referred more than one runner to your office.  Different courses for different horses is how I see our views on orthotic decievs.Brian Fullem                ]]></description>
